Pursuant to the requirements of the Law of the Republic of Kazakhstan "On Personal Data and Protection Thereof", I hereby give my unconditional, specific and informed consent to Kcell JSC BIN 980540002879 (“Company), as well as to organizations having civil law relations with the Company, for the collection and processing of my personal data (including future changes and (or) amendments thereto) recorded on electronic, paper and any other media (“Personal Data”) provided via www.activ.kz, www.kcell.kz websites, the Company's mobile applications (and not only) or in my personal presence, and confirm that by giving such consent I am acting of my own free will and in my own interests.

In this Agreement, organization is understood to be the following company which provides additional services (Providers) INTECH VAS LLP (BIN: 111240018163);

Consent to the collection and processing of my Personal Data is given for the Company to (including but not limited to)

By this consent I authorize the Company and Provider to perform actions, including by utilization of automation tools, information systems, Internet resources, various services and software in relation to my Personal Data, that are necessary to achieve the above purposes, including (without limitation): recording, copying, backup, protection, screening, collection, systematization, accumulation, storage, clarification (updating, modification), retrieval, transfer/receipt (dissemination, providing access) to/from legal/official sources, depersonalization, deletion, confirmation and verification of any kind of Personal Data, including detection of any discrepancies/changes and additions to the existing Personal Data, use, distribution, blocking, destruction, transfer of Personal Data to third parties in the territory of the Republic of Kazakhstan, trans-border transfer to the territory of foreign countries (except for the customer proprietary network information) in accordance with the national personal data protection regulations of these states, as well as the implementation of any other actions in respect of my Personal Data, subject to compliance with the laws of the Republic of Kazakhstan. Transfer of the customer proprietary network information outside the Republic of Kazakhstan is prohibited.

I am aware of the right to withdraw this Consent at any time, except in cases of contradiction to the requirements of applicable law or the existence of outstanding obligations to the Company and/or Provider. From the moment of receipt of the withdrawal, the Company undertakes to cease all actions on collection and further processing of personal data provided by me in accordance with the purposes of this Consent.

The period of storage of personal data is determined by the moment of achievement of the purposes of their collection and processing.

I am aware that I have the right to withdraw this Consent at any time, unless such withdrawal goes against the applicable laws, or I have outstanding obligations to the Company and/or Provider. Upon my withdrawal of this consent, the Company undertakes to cease any and all actions related to the collection and processing of personal data provided by me for the purposes stated in this Consent.

The period of storage of my personal data will be limited to the time when the purposes for which it is collected and processed are achieved.

I confirm that I have read and agree with the Company's Policy on Collection, Processing and Protection of Personal Data and confirm the accuracy of the data I have provided, such as mobile number and other information I have provided to the Company and the above mentioned third parties for the above purposes.

I have hereby agreed with the Company and third parties to consider this consent form as the written form of a transaction, including those made by exchange of electronic messages (including, but not limited to, exchange of SMS via a mobile number registered in my name). This consent shall be valid and applicable to the relations between me and the Company, me and third parties arising from the moment of registration in the Company's database.
